The Medical Board of California has ordered the revocation of Dr. Mark Daniel Cook’s medical license after an administrative law judge found that he practiced medicine while suspended and violated probation requirements.
The revocation is scheduled to take effect at 5 p.m. Friday, July 31. Cook was also ordered to pay $135,076.25 in investigation and prosecution costs.
The decision found that Cook practiced medicine on at least three occasions while prohibited from doing so. The incidents involved a patient’s methadone prescription, a medical imaging order signed under another physician’s name and tramadol provided without required labeling or records.
The board also found violations involving the structure and operation of Cook’s medical business, which did business as Oakdale Family Medicine.
Cook denied treating patients during his suspension and said he performed only administrative and scribe duties. The judge found his testimony was not credible and concluded that revocation of his medical license was necessary to protect the public.
The matter was an administrative licensing case, not a criminal prosecution.
