Stanislaus County Public Health’s COVID-19 Vaccine Clinics will be open for individuals identified in Phase 1A and seniors aged 65 and older. Vaccines are available to Stanislaus County residents only. If you reside in a different county but work in Stanislaus County, you may be eligible to receive the vaccine if your job is identified under the current Phase/Tiers.

Vaccination screening forms will be provided at the clinics. If you want to fill out the screening form prior to arriving at the clinic, you can download a copy from the Stanislaus County Health Services Agency website at schsa.org.

In Oakdale, the Gladys L. Lemmons Senior Community Center will be the site for a ‘First Dose Only’ vaccination clinic on Thursday, Feb. 4 from 9 a.m. to 4 p.m., at 450 E. A St., Oakdale.

Proof of residency is required, such as a rental/lease agreement, a deed/title to residential real property, mortgage bill, utility or cell phone bill, medical document, insurance document and more. A full list can be found on the Health Services Agency website.

At Modesto Centre Plaza, there is a ‘Second Dose Only’ clinic scheduled from 9 a.m. to 5 p.m. on Friday, Feb. 5 at 1000 L St., Modesto. Parking will be available at both City-operated garages on 9th and on 10th Street.

Stanislaus County officials said the clinic locations and hours will be updated weekly or as vaccine availability changes.
