Authorities reported the death of a Modesto man on Sunday morning, Nov. 17 in a crash that occurred east of Oakdale.

According to the California High Patrol, the accident occurred on Highway 108 east of Lancaster Road about 8:45 a.m.

Stanislaus County Coroner’s Office personnel on Monday identified the fatally injured motorist as Hormoz Azizpor, 57, of Modesto.

According to the CHP report, the driver of an eastbound Nissan Rogue, identified as 83-year-old Hector Ramos Jose of Oakdale, collided with a westbound Toyota pick-up truck driven by Azizpor. Authorities said for an unknown reason, Jose’s vehicle drifted into the oncoming lane and the two vehicles struck head on.

Azizpor suffered fatal injuries as a result of the head on crash and a passenger in his truck, a 35-year-old woman whose name was not released, was taken by a life-flight helicopter to Memorial Hospital in Modesto with major injuries.

Jose, who also suffered major injuries in the Sunday morning crash, was taken by Memorial via ground ambulance.

Authorities said it appeared that Azizpor was not wearing his lap and shoulder seatbelt correctly at the time of the crash and remind motorists that “proper usage of seatbelts can save lives.” Both the female passenger and Jose were wearing their seatbelts.
